Many forms are available at the Office of the Registrar. The student will need to visit the Office of the Registrar to fill and process the following:
1. Request for Transcripts, Certificates and Documents
2. Petition Form (E-Petition via SIS)
3. Registration and Drop/Add Forms
4. Cross Registration Form
5. Non-Transferable Forms (E-Petition via SIS)
6. Graduation Form (Accessible through the SIS)
7. Student Schedule Sheet
